  • Description of Incident: A CVS Pharmacy in Nottingham, MD informed Dr. David M. Cromwell that a patient of Dr. David M. Cromwell had a prescription for Oxycontin filled on October 1, 2010 and then tried again on October 13th to have a prescription filled for Oxycontin on one of the doctor’s prescription forms. The pharmacy called Dr. Cromwell’s office on October 13th to verify that Dr. Cromwell had written the prescription, which he had not. Dr. Cromwell believes that this person stole his prescription pad when in the office for a patient visit.

  • Description of Incident: Received information at 10:30 AM 11/21/11 from Michelle, Pharmacist at Harlem Gardens Pharmacy in Baltimore that an individual tried to fill a fraudulent prescription for Oxycodone at her pharmacy. Michelle advised that the script appeared authentic. After viewing faxed image of script it was determined that an unknown quantity of Dr. Ewen’s prescriptions had been stolen. Copy of prescription attached. Police were notified.

  • Description of Incident: ON 9/23/11 I WAS CONTACTED BY A PHARMACIST REGARDING A FALSIFIED/ FORGED OXYCODONE 30 MG PRESCRIPTION WRITTEN ON A STOLEN PRESCRIPTION PAD FROM ST AGNES HOSPITAL CANCER INSTITUTE. THE PERPETRATOR, WHO IS NOT A PATIENT OF MINE, WAS ARRESTED AND TRIAL IS PENDING.

    ON 11/3/11 I WAS CONTACTED BY ANOTHER PHARMACIST REGARDING A FALSIFIED/ FORGED OXYCODONE 30 MG PRESCRIPTION WRITTEN ON A STOLEN PRESCRIPTION PAD FROM ST AGNES HOSPITAL CANCER INSTITUTE. THE PERPETRATOR, WHO IS NOT A PATIENT OF MINE, WAS ARRESTED.

    THE SAME PRESCRIPTION PAD ALSO HAS THE NAME AND DEA# OF SYLWESTER DZIUBA, M.D.

    DR. DZIUBA AND I WOULD LIKE PHARMACISTS TO PLEASE CALL OUR OFFICE AT 410 368 2965 TO VERIFY ALL CONTROLLED SUBSTANCE PRESCRIPTIONS. WE ARE ON CALL 24/7, BUT IF WE ARE LATE IN RESPONDING THEN WE ASK THAT YOU NOT FILL SUCH PRESCRIPTIONS UNTIL CONFIRMATION WITH OUR OFFICE EVEN IF IT’S NOT UNTIL THE NEXT BUSINESS DAY.
