Get Ready for April 2012!!
Please review the posted RAI manual for April 2012 use.  Errata documents and page replacements have
already been posted (see below) for this manual which is to be used starting April 1, 2012.  Please note that sections K, M and O have had some adjustments.  Chapters 2, 5 and 6 also have some modifications.
We anticipate some provider training to be held in various areas around the state during April –June.
Those training opportunities will be posted to this site as soon as they are finalized.

What's New:
March 12, 2012
 
A CORRECTED MDS 3.0 RAI Manual (v1.08) Errata document, "MDS 3.0 RAI Manual (v1.08) V3 Errata_March_12_2012" has been posted under the download section of this page. The document that was posted on March 6, 2012 contained correct information about the identified issues and resolutions; however,
the replacement pages for the manual were not updated. This errata document contains the replacement
manual pages and describes and corrects seven issues that are present in the MDS 3.0 RAI Manual (v1.08), effective April 1, 2012. This document also contains updated pages from the manual that were affected by
these corrections. The footer on each of the affected pages is appended with an "(R)" to reflect that it has
been revised.
 
Section S for Maryland
Changes for Section S will only be accepted 1 time per year per new CMS requirements, no change is
planned for April 2012.

Change in MDS Correction Policy
Effective with submissions on April 1, 2011, CMS will no longer allow facilities to modify the MDS 3.0 reason for assessment items, assessment reference date (ARD) discharge date and entry date. If a modification record is submitted attempting to change any of these items, the record will be rejected.  These MDA items must be corrected by inactivating the old record and submitting a new record.  Please refer to Chapter Five in the RAI Manual as revised to reflect this change.

The MDS Provider User’s Guide is available under the MDS 3.0 link on the QIES Technical Support Office website.  This guide provides detailed information for the submission of MDS 3.0 records by nursing home and swing bed providers.  There is also a webpage for Vendor MDS Information including question and answer documents from CMS/Vendor teleconferences.
 
A draft MDS 3.0 Quality Measure Technical User’s Manual is available on the CMS MDS 3.0 Technical Information web page.

Important MDS 3.0 Update Required:(Posted 02/27/2012)
MDS 3.0 requires new items on assessments with a Target Date of 04/01/2012 and later. Failure to upgrade
your software will result in fatal errors. Contact your software vendor for a software upgrade prior to encoding assessments with a Target Date of 04/01/2012 and later. The enhanced software should incorporate the
changes contained in the MDS 3.0 Submission Specs V1.10.1, which can be found at: http://www.cms.gov/NursingHomeQualityInits/30_NHQIMDS30TechnicalInformation.asp
jRAVEN users will find jRAVEN 1.1.4 available for download after 03/27/2012 at https://www.qtso.com/ravendownload.html. This upgrade incorporates the changes beginning 04/01/2012.

I. SUMMARY OF CHANGES: Pub 100-02, Chapter 8, section 40.1 is being revised to add physician assistants to the list of practitioners that can perform the required initial certification and periodic recertifications of the
need for an SNF level of care.
 
EFFECTIVE DATE: Effective with items and services furnished on or after January 1, 2011, consistent with section 3108 of the Affordable Care Act.

CMS has posted a revised MDS 3.0 RAI Manual on June 2, 2011 and Section M and Appendix A were updated
in July 2011. The RAI manual is available as a zip file and contains the complete manual and all chapters/sections/appendices/title page/table of contents and forms.

Technical Resources
Change from AT&T to Verizon for connecting nursing homes and home health agencies to the MDS and OASIS Systems expected by October 1, 2011. Verizon will be providing a clientless access solution that does not require installing software. CMS additional information is available on the QIES Technical Support Office website at

Only OBRA and Medicare PPS Assessments Allowed to be Submitted
On the January, 2011 SNF Open Door Forum conference call, CMS explained only MDS 3.0 assessments that are OBRA assessments or Medicare Part APPS assessments should be submitted to the QIES ASAP MDS System. Assessments completed for Medicare Advantage Plans or for other insurance purposes should not be submitted. CMS identified this as a change from MDS 2.0, which allowed submission of all MDS assessments. Vendors may attempt to provide support software to assist providers in managing this process.

 "FAQ and CMS Clarifications- MDS 3.0 is evolving-- the items are current as of the date they are posted. Please check back for updates. Every effort will made to ensure information is accurate.

 

Interviews:
At this time CMS has clarified that each interview is applicable to only one assessment. For example, a 5 day PPS assessment interview is done on day 7 and the admission assessment has an ARD on day 13. Even
though the prior interview is in the 7 day look-back their should be 2 sets of interviews, one for each
assessment.CMS clarification: Attempt to conduct interviews with all residents regardless of item scoring at B700. While a resident might be rarely or never understood, he or she may be able to answer questions at the point in time interview.

 

Section A:
A1000- Ask the resident or responsible party to determine the appropriate race/ethnicity category. You may check more than one item.

 

A2400- Medicare Stay- the start date of the Medicare stay is the date the Medicare started in the FACILITY. Hospital stays or stays in other facilities are not considered. Only Medicare stays starting on or after January 1, 2010 should be included.
Section F:
This section is found on comprehensive assessments only.
Section G:
Count the ADL tasks only if completed by facility staff.
